Myler Disability collects information through our web site forms, located throughout the site. Within the website we collect only your name, email address, age range, phone number, whether or not you have seen a doctor, and whatever notes you elect to leave us.

Part II. Information Usage

The information collected by Myler Disability will be used for giving feedback to the providers of the information as to their Social Security and/or Disability applications. Users who provide information will receive a confirmation that the information was received and will receive a response from Myler Disability concerning potential coverage options.

The information we collect will not be used to create customer profiles, only as needed to file and/or appeal your Social Security Disability and/or SSI applications. We will not supplement information collected at our web site with data from other sources without the consent of the person providing the information.

Myler Disability will not share data with anyone except the government agencies necessary to process your applications, and then only with your express permission.
